At Girudala, we are committed to supporting our community to manage long term health conditions through culturally safe and holistic care.
Our Chronic Care Services are designed to help people living with ongoing health issues such as diabetes, heart disease, respiratory conditions and other chronic illnesses. We are an accredited practice and provide bulk billing for eligible patients.
Girudala’s health worker led chronic care program supports people and families to understand their health, stay on track with treatment, and feel confident managing their wellbeing.
Our care is respectful, culturally safe and grounded in relationships.
